Introduction

We take your privacy very seriously, and we are committed to protecting the privacy and security of your information.   This Privacy Policy describes how we collect, use and share your information required to provide you with the services provided on Homecare Hub. 

Information we collect from you

Information you provide to us 

Account and User Profile Information

When you sign up for a Homecare Hub Account, we require that you provide certain information such as your first name, last name and email address.  Optional information includes a profile picture and phone number.

Patient Information

In order to provide homecare providers with enough information to provide the needed services, accurate patient information is required.  This includes the patients first name, last name, date of birth, phone number, and address.  Optional information includes the patient's email address, diagnosis and any other additional information you feel is relevant for the homecare provider.

Hub Information

To help create Homecare Hubs, we require Hosts and users to share certain information.  This includes Hub Details such as required services and number of members, Address, and Schedule, along with patient details such as name, age, gender, diagnosis (to match patients with similar diagnosis), interests, preferred language and other information provided by the user.

Care Provider Information

Care providers provide information about their business, such as name, address and contact information, as well as the services they provide including pricing.  

Payment Information

To user certain feature of the Homecare Hub platform (such as booking care or joining a Hub), we require you to provide certain financial information, such as your credit card information, to facilitate the processing of payments.

Communications

Communication between users, patients, hubs, hub members and agencies are collected, along with any information that is voluntarily provided, to facilitate these communications and understand the nature of the communications to further improve the user experience.

Information provided to us from Third Parties

Agencies on the Homecare Hub platform provide information about users and patients such as comments, assessment requests, schedule, billing information and any other information used to provide quality service to patients.

How we use information we collect

We use the information we collect to

· Connect patients with care providers;

· Provide, improve, expand and promote the Homecare Hub platform;

· Facilitate communication between patients and Care Providers;

· Facilitate transactions and payments;

· Provide you with customer support;

· Communicate with you, either directly or through one of our partners, including for marketing and promotional purposes;

· Personalize the experience for users and patients;

· Help match patients to Hubs

How we share the information we collect

Sharing between users

Some basic, non-identifying information that you provide is shared with other users hosting, joining or requesting to join a Hub. This includes information such as your first name, postal code (to show/use your general location), diagnosis, care required (PSW, Nurse, etc.), interests, preferred language, gender, age, and preferred schedule.

Sharing between users and Care Providers

User Profile, Patient information and communications are shared with Care Providers to allow them to provide the requested services. Care Provider information is shared with users and patients to allow them to search the Homecare Hub platform, request and use the provided services, and communicate with Care Providers.

Other

Location Information

While you can prevent your browser and/or device from sharing location information at any time through your Browser and Device’s  settings, Patient and Care Provider location is core to the Homecare Hub Platform and without it the services can not be provided.

Editing and Accessing Your Information

Users and Care Providers can review and edit certain account information by logging in to your account settings and profile.  If you would like to terminate your Homecare Hub account, please contact us with your request. If you choose to terminate your account, we will deactivate it for you but may retain information from your account for a certain period of time and disclose it in a manner consistent with our practices under this Privacy Policy for accounts that are not closed. We also may retain information from your account to collect any fees owed, resolve disputes, troubleshoot problems, analyze usage of the Homecare Hub Platform, assist with any investigations, prevent fraud, enforce our Terms of Service, or take other actions as required or permitted by law.

Data Security

We are committed to protecting the data of all users  on Homecare Hub platform. While reasonable precautions have been taken to protect your data, no security measures can be 100% secure, and we cannot guarantee the security of your data.
